I understand that there are inherent risks associated with nail care services. These include and are not limited to irritation or discomfort on or around the nails, injury, and rarely, infection.

I understand that maintenance appointments are recommended every 2-3 weeks and are vital to maintaining the quality and integrity of the nail and its applied enhancements.

I understand that I will be required to sit in an upright position with minimal upper body movement for the duration of the appointment, except for one planned break at the second hand-washing step during service. The second hand-washing step is generally around the halfway point of the appointment. Expect a single sitting session to range from 30 minutes to 3 (three) hours. If I am unable to meet this physical requirement, I understand that I should not schedule services.

I understand that I should reschedule my appointment if I am showing signs of injury that interferes with the provided services. I understand that I should reschedule my appointment if I am showing signs of infection, if I have been exposed to a spreadable infection, or if I have been told by a medical provider that I may be able to spread an infection.

I understand that the total cost outlined in my appointment confirmation is an estimate and that the total amount due at the time of service may change depending on the actual services I receive.

I understand that all sales are final. 

I understand that this agreement is binding and agree that I have read and fully understand all information above and expressed in the Nebula Nails Policies. 

I release my technician, Mercedes Murphy, from all liability associated with the services rendered. This agreement will remain in effect for this procedure and all future procedures conducted by my technician.
